Clinical Nurse Specialist/Case Manager

UnitingCare Ageing Mirinjani Village is seeking an experienced Clinical Nurse Specialist/Case Manager to work fulltime hours Monday to Friday at our state of the art facility located in Weston, ACT

The successful applicant will be responsible for ensuring that high quality person centered care is provided for all residents and will act as a key clinical resource for staff management and care planning

Essential Criteria:
Current Certification with the ACT Nurses’ Registration Board

5 years post graduate experience in health and Aged Care

Solid understanding of legislation in relation to Aged Care
Demonstrated knowledge and commitment to a Person Centered Model of Care

Desirable Requirements:– Well-developed understanding of and demonstrated ability to undertake the functions of the case management process as relevant to clients with complex care needs
 Demonstrated understanding and knowledge of the needs of the clients of the Home and Community Care target group, the concept of packaged care and the use of brokerage funds
